AgeWell At Home

Seniors often want to remain at home independently but are in need of services. When people ask “how can I keep my father in his own home?” or “what services are there to keep my mom safe?”, we can refer to AgeWell at Home. This service can help seniors access the services they need in an easy one-stop access point.

Care Coordination

Care coordination (also known as care management) is a service that allows older adults to remain in the community as independently as possible. The service begins with an in-depth assessment by a licensed clinical social worker.

Caregiver Connection

Caregiver Connection has the professional staff and experience to help you find solutions and make decisions related to care for yourself or a loved one. We take the worry out of hiring a trusted, responsible and compassionate caregiver. You are in charge: you choose the hours you need and the caregiver you want. Caregiver wages are negotiated directly with you.

Caregiver Support Group

A Caregiver support group is a place where family caregivers are welcome to come and learn from one another, lend support to one another, and help each other through the stresses of caregiving. It is a confidential space for all issues regarding giving care to another person in your life.

ElderAlert

Falls are the greatest threats to independent living, but a personal emergency response system, called ElderAlert, can provide immediate assistance. At the touch of a button on a pendant or bracelet, you can be in touch with an operator who can contact an ambulance, fire department, or police officer in case of an emergency.

Home Health Services

As people age, they can become more frail and less likely to go into the community to access services. Home Health Services offers care to patients in their home, such as patients that have difficulty leaving home due to medical or physical restriction, or have returned from hospital stays and would benefit from home health services.

In Home Safety Assessment

Home Safety Evaluations help prevent household accidents, alleviate hazards and protect the home against potential problems by recommending home improvements, modifications, and repairs. Work can range from replacing doorknobs with pull handles and adding grab bars in bathrooms to installing a wheelchair ramp or a stairglide.

Mental Health Services

JFCS Counseling offers a wide range of mental health services for seniors provided by caring and experienced clinicians. Some of our services include Geriatric Psychiatry and Geriatric Psychotherapy.

Outpatient Rehabilitiation

Access to various types of therapies can help healthy seniors stay healthy and less healthy seniors become stronger. With two locations to choose from, adults of all ages can gain access to physical, occupational and speech therapies.

Palliative & Hospice Care

Bereavement Support Group

Loss is a part of many older adults’ lives and we are here to support them in their time of need. The bereavement support group provides a safe, confidential place for those who are grieving to process and share their feelings, concerns and coping strategies with others who are also trying to cope with the many emotions associated with loss.

Housing

The New Riverview

The New Riverview is a housing community located on the Monongahela River, just off Browns Hill Road in Squirrel Hill. Riverview’s two buildings sit on acres of manicured grounds, with 191 renovated studio, one- and two-bedroom apartments for all income levels.

Weinberg Terrace

Weinberg Terrace is a licensed personal care facility located in the heart of Squirrel Hill. It is in walking distance to the Murray-Forbes shopping corridor where banking, post office, and shopping are all within reach. Each apartment is equipped with a kitchenette, full bathroom, and spacious living areas.
